What Gets Tracked, and Where It Lives

Every document category is attached to the record it belongs to — a staff member, a person served, or both. Your organization’s profile decides which categories are active.

Who It’s For

Many Provider Types. One Platform.

SentroCare is built around a requirements engine rather than a single license type. Your organization’s profile determines which rules apply — so the platform grows with the licenses you hold.

Requirement sets per license

Each provider type carries its own document requirements, lead times, and review cadences. Selecting your type activates the right set — nobody on your team maintains a checklist by hand.

  • Documents mapped to license conditions
  • Cadences and lead times per category

Every location in one view

Directors overseeing several sites stop stitching together a binder per location. See where the whole organization stands at once, then narrow to a single site, program, or person when something needs attention.

  • Organization-wide status at a glance
  • Filter to a site, program, or person

Multi-license organizations

Operating under more than one license is the normal case, not an exception. Run a combined compliance profile in a single account and let each record follow the rules that apply to it.

  • One roster across license types
  • Per-record rule resolution

245D HCBS Providers

Minnesota Statute 245D

Home and Community Based Services providers delivering waiver services — background study tracking, client rights documentation, service plans, and annual reviews.

Home Care Agencies

MN Rules Chapter 4668

Licensed Home Care and Temporary Home Care agencies — aide competency, supervised hour verification, service agreements, and renewal timelines.

Additional Minnesota provider types are onboarded as configuration. If you hold a license that isn't listed, tell us during your demo and we'll walk through what setting it up involves.

Security

Your Clients' Information Is Safe

Protecting the people you serve — and your organization's license — isn't an add-on. It's how the platform is built.

Protected Client Records

Sensitive client and staff information is shielded with multiple layers of protection. Even in the unlikely event of a breach, your data stays unreadable.

Legally Valid Signatures

Every signature meets Minnesota and federal requirements for electronic signing — just as binding as a handwritten one.

Role-Based Access

Front-line staff, compliance managers, and administrators each see only what their job requires. Nothing more.

Permanent Audit History

Every change is recorded with who made it and when. Nothing to reconstruct when DHS asks what happened.

Secure Data in Transit

All information moving between your team and the platform is encrypted end-to-end and cannot be intercepted.

Controlled Document Access

Documents are never publicly reachable. Every download is authorized and expires — files exist only when requested.

HIPAA-Ready Infrastructure

Built to meet HIPAA requirements from the ground up, not patched on afterward. A BAA is available on request.

Always Up to Date

The platform updates continuously and securely, without downtime. Your team is always on the latest release.

How It Works

Operational in About a Week

No lengthy implementation. No professional services engagement. Three steps from signup to a compliance program that maintains itself.

01

Configure your organization

Select your provider type, add service locations, and set your compliance profile. The requirements engine uses this to determine which documents apply to each staff member and client — automatically.

02

Generate and distribute

Add staff and clients. Applicable documents are identified, generated as PDFs, and sent for signature in one workflow. Clients receive a link — nothing else is required on their end.

03

Track and stay ahead

Monitor completion in real time. The expiration scanner runs continuously in the background, alerting your compliance team before any deadline is reached.

FAQ

Questions, Answered

Common questions from compliance administrators and agency operators.

Which provider types does the platform support?

245D HCBS and Home Care are configured today. During onboarding you select your provider type and the platform automatically applies the right document requirements for your license. Organizations operating under more than one license type can set up a combined compliance profile in a single account. Because requirements are defined as configuration rather than code, additional Minnesota provider types can be set up without a separate product — if you hold a license that isn’t listed, raise it during your demo and we’ll walk through what that involves.

Do clients need to create an account to sign documents?

No. Your client receives a link and signs directly from their phone, tablet, or computer — no account, no app, and no paperwork to print or mail. The link is only valid for 72 hours for security, and every signature collected is legally valid under both Minnesota and federal law.

How is sensitive client and staff information protected?

Sensitive information is protected with multiple layers of security built into the platform from day one — not added on afterward. Records are kept protected inside the database, meaning the information is unreadable to anyone without proper authorization. Documents containing sensitive information are only accessible via secure, time-limited links. We’re happy to walk through our security practices in detail during a demo, and a HIPAA Business Associate Agreement is available upon request.

How long does implementation take?

Most organizations are fully up and running within a week. No outside consultants or lengthy setup projects are required. The onboarding flow guides your team through configuring your provider type, adding service locations, selecting your document templates, and inviting staff — step by step.

Can the platform be hosted in our own environment?

Yes. If your organization requires complete control over where your data lives, SentroCare can be deployed entirely within your own hosted environment. This gives you full data ownership and the ability to sign a Business Associate Agreement directly with your infrastructure provider. Ask us about this option during your demo.

What happens when a certification is about to expire?

The platform continuously monitors every certification and document deadline across your staff roster. Your compliance team receives email alerts before any deadline — not after a lapse has already happened. You can configure how far in advance alerts are sent and who receives them, on a per-document basis.
